We can provide case management, meaning providing continuous evaluation of the patient and their family and their changing needs and make every effort to meet them and network with doctors and other community agencies.

We also provide education. Hospice gives families as much information as possible about their loved one's illness that teaches them how to be an effective caregiver as well as informs them of the emotional aspects of caring for their loved one.

Hospice also provided resources. We can provide medical equipment and materials to assist in care as well as emotional, spiritual and social support for the patient and their family.

Volunteers are another important aspect of our care team. We will work together to make the perfect fit with a patient, their family and the volunteer. They can assist with hands on care, read or visit with the patient, support the family with many different tasks.

All of the services we offer are always free of charge and are based on the individual, no person is the same and no situation is the same. We will create a care plan that is right for you and your unique needs.
